Did you know?

You can access official immunization records through the secure Kentucky Immunization Registry (KYIR) Public Portal.  This is a free service of the Kentucky Department for Public Health that allows individuals, parents, and legal guardians to access official immunization records from KYIR.  The portal enables residents to access, save, and share their vaccination records directly from KYIR, a web-based system used by health care providers to report vaccines as they are administered.

Portal users may choose to receive a PDF of their complete immunization record or school certificate.  Vaccines administered by physicians who do not utilize the registry will not be included in the KYIR record.   Portal users can also access COVID-19 vaccine records in the form of a PDF or as a SMART Health Card, a digital copy of your COVID-19 vaccine record that can be stored on your smartphone as a QR code.
